INSTRUCTIONS:
OFF RESERVE MEMBER(S):
1. Please complete the attached application form and submit a legible copy of any (2) pieces of identification: Treaty Card or Valid Government I.D., and any (one) of: Provincial Health Card, Social Insurance Card, Birth Certificate.
2. To have a cheque mailed: please ensure your full address, including postal code and email address is completed on the form.
3. Payments will not be distributed to other parties. You are responsible to receive your own payment, NO EXCEPTIONS.
MINORS (UNDER 18 YRS OF AGE)
1. The Payment will be distributed into the Minor’s Trust Fund in the new year.
2. The Ochapowace Minor must be registered before Friday, December 6, 2019 to receive payment.
3. Walmart gift cards will be provided to registered Nation minor(s) at the stated locations. Please have any (one) of: provincial health card, social insurance card, birth certificate to receive a gift card.
For all other Nation members able to attend the Christmas Locations in person, please see the following information:
IN PERSON (OVER AGE OF 18 YRS.):
1. Please complete the form (Forms will be provided at the Christmas Dinner Dates/Locations)
2. Present (2) pieces of Identification: Photo Identification Treaty Card or Valid Government I.D., and any (one) of: provincial health card, social insurance card, birth certificate.
3. If you CAN NOT provide photo identification the following is required: - Complete the Guarantee Declaration portion (on the same application form), it will be signed and approved by any one of Council or Representative(s) on site. - Your photograph will be taken and attached to the signed Application Form and photocopy of identification(s) provided by you on site
4. Payment will not be released to other parties. You are responsible to receive your own payment, NO EXCEPTIONS.
